Will you share information from my consultation with my NHS GP?

With your agreement, relevant information may be shared with your NHS GP to support continuity and safety of care. There may also be circumstances where information needs to be shared without your consent, for example where there is a serious concern about your safety or someone else's safety, or where sharing is otherwise required or permitted by law.
